Cavities can happen even before a baby has his first piece of candy. This was the difficult lesson actor David Ramsey of the TV shows Arrow and Dexter learned when his son DJ’s teeth were first emerging.
“His first teeth came in weak,” Ramsey recalled in a recent interview. “They had brown spots on them and they were brittle.” Those brown spots, he said, quickly turned into caviÂties. How did this happen?
Ramsey said DJ’s dentist suspected it had to do with the child’s feedings — not what he was being fed but how. DJ was often nursed to sleep, “so there were pools of breast milk that he could go to sleep with in his mouth,” Ramsey explained.
While breastfeeding offers an infant many health benefits, problems can occur when the natural sugars in breast milk are left in contact with teeth for long periods.Â Sugar feeds decay-causing oral bacteria, and these bacteria in turn release tooth-eroding acids. The softer teeth of a young child are particularly vulnerable to these acids; the end result can be tooth decay.
This condition, technically known as “early child caries,” is referred to in laymen’s terms as “baby bottle tooth decay.” However, it can result from nighttime feedings by bottle or breast. The best way to prevent this problem is to avoid nursing babies to sleep at night once they reach the teething stage; a bottle-fed baby should not be allowed to fall asleep with anything but water in their bottle or “sippy cup.”
Here are some other basics of infant dental care that every parent should know:
- Wipe your baby’s newly emerging teeth with a clean, moist washcloth after feedings.
- Brush teeth that have completely grown in with a soft-bristled, child-size toothbrush and a smear of fluoride toothpaste no bigger than a grain of rice.
- Start regular dental checkups by the first birthday.
Fortunately, Ramsey reports that his son is doing very well after an extended period of professional dental treatments and parental vigilance.
“It took a number of months, but his teeth are much, much better,” he said. “Right now we’re still helping him and we’re still really on top of the teeth situation.”

Despite everyone’s best efforts, one of your child’s primary (“baby”) teeth has become decayed to the point it might be lost prematurely. Saving it would require extensive treatment like capping it with a crown or performing a pulpotomy, similar to a root canal treatment.
You may be thinking: since it’s going to come out eventually, why go to the expense of trying to preserve it longer? Actually, there are good reasons to save a baby tooth depending on your child’s age — for now and for the future. Here are 4 of them.
They’re important for nutrition. Baby teeth are quite similar to permanent teeth — not only do they look like them, they perform like them too, enabling a growing child to chew and digest food needed to boost their development. Even the loss of one tooth for an extended period makes effective chewing harder.
They’re important for speech development. With their first words, children develop speech patterns rather quickly. Their baby teeth play an important role in this: just like permanent teeth, they provide the tongue with points of contact for making a variety of sounds. A missing tooth for a prolonged period could interfere with making certain sounds and could have a stunting effect on their speech development.
They’re important for permanent teeth eruption. Baby teeth also serve as placeholders for their successors, the permanent teeth that are in development just under the gums. A baby tooth normally remains until the permanent tooth is ready to erupt within the path set by the primary. If they’re lost prematurely, the permanent tooth may not erupt as it should; and adjacent permanent teeth can drift toward the empty space and out of alignment.
They’re part of their smile. Baby teeth help children fit in socially with adults and other children — they help them look normal. A missing tooth stands out when they smile — and not in a good way. This could impact the way they interact socially with others, extending even into adulthood.

For many parents, the image of an infant intently absorbed in sucking a pacifier — or her own thumb — is one of the cherished memories of babyhood. But if this habit goes on for too long, it can cause problems with the child's bite. Want to know what the potential predicaments are, when you should be concerned about the behavior, and what you can do? Read on!
Thumb sucking is a natural, comforting behavior of humans (and some other primates) related to nursing. It usually goes away on its own by the time the permanent teeth are coming in. But it can be a hard habit to break — and if it becomes a persistent behavior, the consequences may include a problem called an “open bite.”
In a normal bite, the top teeth slightly overlap the bottom teeth. When the thumb (or any other object) constantly rests between the upper and lower teeth, however, the pressure it exerts may prevent the teeth from fully erupting (coming out from the gums into the mouth) and alter the shape and development of the upper and lower jawbone. This result is a gap between the upper and lower teeth.
The same problem may also be caused by prolonging the “infantile swallowing pattern,” a forward-thrusting position of the tongue which, like thumb sucking, normally begins to cease around age four. That's when it is replaced by the adult swallowing pattern, where the tongue is held behind the teeth, against the roof on the mouth. Researchers believe that most open bites result from the failure to change from the infantile to the adult swallowing pattern.
When should you be concerned about the thumb sucking habit? If the behavior continues much past toddlerhood, or if the sucking is particularly active, you may wish to have us evaluate your child's bite. The American Academy of Pediatric Dentistry recommends having the habit stop by age 3. Persistent thumb sucking can actually push the teeth forward and change the growth patterns of the jaw, creating more difficult problems.
There are several methods for controlling the behavior and correcting problems with the bite. One is an appliance called a “tongue crib.” This thin metal device is placed behind the upper and lower incisors. It discourages thumb sucking, while at the same time helping to keep the tongue from inserting itself between the upper and lower teeth. Eliminating these unhelpful habits is essential to allow the teeth to erupt into proper position and to allow for the normal development of the jawbones.
Recent research has also shown that individualized exercise routines called orofacial myofunctional therapy (OMT) can be highly effective in preventing open bite relapses. These exercises are designed to retrain muscles in the face, tongue and lips, and can help to create good chewing and swallowing patterns.

Even before your infant's first tooth emerges, you can take steps to reduce the risk for cavities!
Cavities occur when decay-causing bacteria living in the mouth digest carbohydrates (sugars) introduced into the mouth via food and beverages. This produces acid, which can eat through the protective enamel surface of teeth and attack the more vulnerable dentin below. Infants aren't born with decay-promoting bacteria; however, they can acquire them from their caregiver(s) through close contact, for example:
- Kissing on the mouth
- Sharing food
- Sharing eating utensils (e.g., a spoon or glass)
- Cleaning off a pacifier by mouth
Tooth decay is the most common chronic childhood disease! It can start as soon as the first tooth erupts — which generally happens around age 6 to 9 months but can be as early as 3 months or as late as 1 year. Besides being potentially painful, severe tooth decay may cause your child to lose the affected primary (baby) tooth before it's due to fall out on its own. That, in turn, can raise the risk of orthodontic problems because primary teeth maintain space for permanent teeth, which also use them as their guide for coming in properly.
It's important to clean your child's teeth regularly once they appear and to refrain from certain feeding activities that have been linked with early tooth decay. For example, use of a sleep-time bottle containing a liquid with natural or added sugars, such as formula or juice, can result in a pattern of severe decay once referred to as “baby bottle tooth decay.” These days, the term early childhood caries (ECC) is more commonly used to also encompass decay linked to continuous sippy-cup use, at-will breast-feeding throughout the night, use of a sweetened pacifier, or routine use of sugar-based oral medicines to treat chronic illness.
We recommend that you schedule a dental visit for your baby upon eruption of his or her first tooth or by age 1. This first visit can include risk assessment for decay, hands-on instruction on teeth cleaning, nutritional/feeding guidance, fluoride recommendations, and even identification of underlying conditions that should be monitored. Your child's smile is a sight to behold; starting early improves the odds of keeping it that way!
